50/50 Raffle Ticket Order Form

Tickets are $25 each or 5 tickets for $100. Winners of the 50/50 Raffle will be drawn at the Annual Meeting Awards Reception on Sunday, November 23, 2008. Winners do not need to be present to win. Remember, the more tickets sold, the greater the prizes!
